Disease Spectrum and Incidence of Breast Lesions in Children: Analysis Based on Ultrasonography

Abstract
Purpose: To analyze the disease spectrum and incidence of breast lesions in children based on ultrasonography in a single institution.

Materials and Methods: From May 2003 to October 2016, 89 patients(mean age 8.3 years, male: female = 7: 82) under the age of 15 years who underwent breast ultrasound were included. The patient¡¯s age, sex, symptoms, treatment method were analyzed. The ultrasound images were retrospectively reanalyzed, and the patient¡¯s clinical course and available pathologic findings were considered together to determine the underlying disease of the breast lesion. We analyzed
the disease spectrum and incidence of each disease. We reanalyzed them into two groups: infantile and young children patients (n=33) and elderly children and adolescents (n=56).

Results: The most common cause of breast lesions in childhood was normal breast parenchyme (n=37, 41.6%). In the infantile and young children, early breast development (n=19, 57.6%) was the most common, followed by lymphovascular malformation (n=7, 21.2%; 6 hemangiomas, 1 lymphangioma), mastitis (n=3, 9.1%), gynecomastia (n=1) and lipoma (n=1). In the elderly children and adolescents, focal breast parenchymal lesion (n=26, 46.4%; including fibroadenomas [n=10], juvenile fibroadenoma [n=6], and phyllodes tumor [n=3])was the most common, followed by prominent normal breast parenchyme (n=18, 32.1%), gynecomastia (n=2), pseudotumor (n=2) and mastitis(n=1).

Conclusion: Breast diseases occurring in pediatrics showed various categories, and the most common was a palpable lump with normal breast tissue affected by early breast development.
